How Housing Supports Health
Stable housing improves physical health, mental health, and overall well-being. Through partnerships with healthcare organizations and community providers, Townspeople helps connect eligible individuals to housing-related services that support long-term housing stability and improved health outcomes.
You may be eligible if you:
- Are experiencing homelessness
- Are at risk of homelessness
- Have frequent healthcare needs
- Are transitioning from a hospital, treatment program, or institutional setting
- Have a disability or chronic health condition
- Are enrolled in Medi-Cal and meet Community Supports eligibility requirements
Eligibility requirements vary depending on the funding source and program. Our team can help determine which services may be available to you.
What Services Are Available?
- Housing Search & Navigation. Helping individuals identify and apply for housing opportunities.
- Move-In Assistance. Helping individuals prepare for successful housing placement and connect with available resources.
- Housing Stability Support. Helping individuals maintain housing, address challenges, and avoid returns to homelessness.
- Shared Housing Opportunities. Helping individuals explore affordable shared housing arrangements through the San Diego Shared Housing Collaborative.
What These Services Do Not Provide
- Do these services pay my rent? Housing & Stability Services help individuals access and maintain housing. Financial assistance may be available through certain programs, but services do not guarantee housing placement, rental assistance, or ongoing rent payments.
- Does Townspeople own all available housing? No. Our team works with a variety of housing providers, landlords, and community partners to help identify housing opportunities.
